General Requirements for Requesting Organizations
Format
- All requests must be in writing, and should be directed to either the Grant Committee of the North Woodward Community Foundation, or any board member.
- Requests must have a cover letter signed by the chief executive or senior development officer of the organization or the chair of its volunteer board.
- Requests initiated by telephone will not be acted upon until a written request is received via mail, hand delivered or e-mail.
- Requests initiated by facsimile are not acceptable.
- Videotapes as supplementary material are not acceptable

- A summary of the organization's mission, goals and major past accomplishments.
- A brief description of current programs and activities, including number of clients, attendance or usage statistics, or any other data regarding quantity of services performed annually.
- The most recent independently audited financial statements or, if not available, the previous year's IRS Form 990.
- The amount of the current year's operating budget, and if applicable, the line item project budget.
- A list of members of the governing board.
- Either the project or the overall program evaluation plan, describing how the organization will judge its success in meeting its goals.
- Donor recognition opportunities and key publicity-related dates.
Ineligible Requests — The North Woodward Community Foundation does not provide support to
- Political parties/organizations or activities.
- Religious organizations for religious (denominational) purposes.
- Organizations that are unable to demonstrate a commitment to equality and diversity (clients, staff and board composition.)
- Student group trips arranged by school districts (except state, national and/or international competitions).
- National or international organizations, unless they are providing benefits directly to service-area residents.
- Projects which may result in undue personal benefit to a member of the Foundation board, or to any North Woodward Community Foundation director or employee.
- Conferences unless for youth 18 and under.
- Single purpose health organizations.
- Medical Facilities, for building or equipment needs.
- General Operating support (e.g. salaries, consultants fees and general office supplies).
- Requests from an Individual Person.
